Add a Design Notes field to the Sketch & Inscription step

In the Sketch & Inscription section of a new order, add a notes field specifically for design notes (flowers, animals, angels, crosses, etc.). Right now this stuff gets lost in the general notes section at the start of the order, and a dedicated field for design instructions would keep it where it belongs — separate from production notes and from general order notes.
