Pick single-grave or double-grave cemetery fee per order

When a cemetery has both a single-grave fee and a double-grave fee saved, MonuDesk applies both instead of letting me pick the one that applies to this order. Pricing isn't size-based and a double isn't 2× a single — these need to be selectable per order.
